Paediatric Health Reports
An assessment of functional health and development, and a corresponding written report; for infants, children and young people up to age 18 years old.
Can be used for personal health records, as supporting evidence for EHCP, Disability living allowance or specialist school application.
The initial assessment involves a complete and age appropriate clinical assessment, including parental questionnaires, and observation through play or other functional tasks.
A thorough report is then compiled, and a follow up appointment organised to jointly review the report.
If desired, appropriate early-intervention plans can be implemented through further follow up, and support provided for onward referral.
