Your data is stored by Southwell Physiotherapy & Sports Injury Clinic.

Our company is hosted on the Wix.com platform. Wix.com provides us with the online platform that allows us to promote our services to you. Your data may be stored through Wix.com’s data storage, databases and the general Wix.com applications. They store your data on secure servers behind a firewall. 

Your data is never passed on, sold or swapped for marketing purposes. 

THE DATA WE COLLECT ABOUT YOU

We may ask for or hold personal confidential information about you which will be used to support delivery of appropriate care and treatment. This is to support the provision of high quality care. Personal details are also collected when you telephone our service with an enquiry however these details are not filed once the enquiry is responded to. We may also collect details from our website should you send an enquiry through our forms. This information is not kept beyond the time to answer the response.

  • Basic details, such as name, address and date of birth.

  • Contact we have had, such as appointments.

  • Details and records of treatment and care, including notes and reports about your health

  • Results of scans, interventions you have had

  • Information from other health professionals or next of kin.

It may also include personal sensitive information such and whether you have a disability, allergies or health conditions. It is important for us to have a complete picture, as this information assists staff involved in your care to deliver and provide improved care, deliver appropriate treatment and care plans, to meet your needs.

Information is collected in a number of ways, via your new patient registration form, your consultant, referral details from your GP or other healthcare professional or directly given by you.

  • To contact you directly in response to any enquiries you have contacted us via email, through our website contact form or by telephone communications.

  • To work effectively with other professionals who may be involved in your care.

  • To review care provided to ensure it is of the highest standard possible.

  • For research and audit ( this would be anonymous case studies).

  • To resolve complaints or issues you may have with Southwell Physiotherapy & Sports Injury Clinic's services.

RETENTION PERIOD OF YOUR DATA

Southwell Physiotherapy & Sports Injury Clinic aligns retention periods for the storage of patient information and records to the NHS Records Management code of practice.

This means we will keep your records for a period of 8 years.

RECIPIENTS OF PERSONAL DATA

To provide best care possible, sometimes we will need to share information about you with others. We may share your information with a range of Health care professionals but will always ask your permission to do this. Information sharing is governed by specific rules and law.

HEALTHCARE REFERRAL

During your treatment you may be referred by your Physiotherapist to another healthcare professional for part of your treatment. Southwell Physiotherapy & Sports Injury Clinic will always obtain permission before undertaking this.

INSURANCE COMPANIES

During your care we may need to share information to insurance companies where insurance plans are used to cover the costs of the care and treatment you as a patient are receiving. The data shared will be in the form of claims relating to the treatment received so that appropriate payment can be made. The insurers that receive this information will be highlighted to Southwell Physiotherapy & Sports Injury Clinic by the individual involved and consent must be given by the patient.

If you would like Southwell Physiotherapy & Sports Injury Clinic to erase your personal data you can do so by formally requesting this via email at reception@southwellphysio.co.uk. There are a few exceptions around this;

  • You have an appointment with us and/or are currently receiving treatment. 

  • You have the right to refuse/withdraw consent to information sharing at any time. We will fully explain the possible consequences to you at such a time as withdrawing consent could include delays in you receiving care.
